in the Independent Practice Physicians Health Network is an affiliation of independent physicians and allied health professionals serving Sheboygan, Calumet, Fond du Lac, Manitowoc, and Ozaukee Counties. Since its inception in 1987, PHN has grown to include over 100 independent, local primary and specialty care physicians and other healthcare professionals who are dedicated to meeting the needs of their patients as unique individuals. PHN services are available in 33 locations in 10 communities in east-central Wisconsin. PHN clearly demonstrates that the independent model of medical practice makes senseboth in excellence of care and cost containment. PHN has significant advantages over other systems represented by large and impersonal corporate models of medical care. PHN emphasizes primary carefamily practice, internal medicine, pediatrics, and obstetrics and gynecology. PHN providers believe that the best medical care depends on the fundamental bond developed between patient and doctor. This relationship allows PHN professionals to know and meet the needs of patients as unique individuals, as whole persons, not merely numbers or charts. PHN is committed to maintaining the highest possible standards of practice and is working continuously to improve quality. Ongoing quality assurance activities include credentialing, physician and staff education, consumer surveys, evaluation of individual office practices, and the development of improved information systems. PHN offers a pattern of medical practice that worksemphasizing preventive medicine and patient education, optimizing appropriate referrals to other specialties of medicine, and utilizing the best acute care and tertiary care facilities and resources available. |
